Failure to provide this information can result in a closing of your Health Savings Account.&nbsp; &nbsp;Often, you may need to provide a copy of your utility bill, a copy of your Social Security card, and a copy of your US passport to establish your identity.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Initial Account Setup<\/h3>\n\n\n\n<p>Go online to&nbsp;<a href=\"http:\/\/secure.optumfinancial.com\/\">secure.optumfinancial.com<\/a>&nbsp;and follow the prompts for Member Sign in and to create your new username and password.<\/p>\n\n\n\n<p>Enter your&nbsp;<strong>First Name<\/strong>,&nbsp;<strong>Last Name<\/strong>,&nbsp;<strong>Date of Birth,<\/strong>&nbsp;<strong>Zip Code<\/strong>, and&nbsp;<strong>Social Security Number (SSN)<\/strong>&nbsp;to begin the identity verification and account creation process. You will create a HealthSafe ID \u2013 which will help keep your account safe and secure by using two-factor authentication.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Adding a Dependent as a Cardholder<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Sign in to your Optum Financial account.<\/li>\n\n\n\n<li>Select \u201cMy Payment Card\u201d from your dashboard.<\/li>\n\n\n\n<li>Select \u201cAdd Cardholder\u201d then order a debit card for anyone listed.<\/li>\n<\/ul>\n\n\n\n<p>If your dependent(s) name is not listed in the drop down, select \u201cAdd New Dependent\u201d.<\/p>\n\n\n\n<div style=\"height:20px\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"spouse\">My spouse also has an HSA<\/h2>\n\n\n\n<p>There is a special contribution limit for married individuals, which provides that if either spouse has family HDHP coverage, then both spouses are treated as having only that family coverage. The form includes detailed instructions\/information, but Optum does advise that members consult with a qualified tax adviser for any assistance needed outside of what\u2019s included on the form.
