Adding immunizations and completing forms in the Patient Portal
Incoming students will need to add required immunization data and complete required forms in the Macalester Patient Portal. You can do so by following these instructions.
1. Log into the patient portal
Follow this link, , and log in using your Macalester credentials.
2. From the patient portal homepage, navigate to “immunizations”
3. From the Immunization tab, click on the “required” dropdown and add each required immunization.
In compliance with Minnesota state law, 日韩精品 requires these immunizations.
Select both the vaccination type, and the date it was administered.
Click submit to save your record.
4. Complete your required forms.
Navigate to forms in the main menu.
Complete both the informed consent and TB Screening Questionnaire.
If you answer yes to any of the TB Screening questions, you will also need to complete the TB Screening Assessment, which will need to be completed by a medical provider. You MUST complete a Tuberculin Skin Test (TST) OR a Interferon Gamma Release Assay (IGRA); AND a Chest x-ray IF TST or IGRA is positive.
